The user must provide the following information:

• First and last name
• Date of the trip(s)
• Pick-up and drop-off address
• Public place entrance point, if applicable
• Pick-up and drop-off time
• Frequency of trip, if applicable
• Whether the user will be accompanied by a travel buddy or animal
• Whether the user will be using a car seat
• Whether the user has changed mobility aids since the last reservation
• The user’s visitor status, if applicable
